Initial Comments:
A facility request was received for a location indicator change via correspondence dated July 15, 2021. The facility will continue to have the legal name as Hoffman Homes Inc. The working name will be changed to Hoffman Homes Inc. Musselman (legal name with changed location indicator).
Hoffman Homes Inc. is in compliance with the requirements of 42 CFR, Part 483, Subpart G Requirements for Psychiatric Residential Treatment Facilities.
